• We are making a carrier change effective 12/1/24 from Connecticare to Cigna. • This will affect the Medical and Dental Insurance. • You only need to complete the payroll authorization form even with the carrier change UNLESS you are adding someone to your coverage or signing up yourself for open enrollment. • The most important things to do for this renewal are: 1. Update doctors and pharmacies of your new insurance information effective 12/1/24 2. Check if any of the medications you take require authorizations with Cigna (generics for Cholesterol are no concern, expensive drugs that have TV commercials require authorizations) details on next page 3. Refill any prescriptions available for refill with Connecticare prior to 12/1 4. If you utilize the mail order program now you will need to fill out new paperwork for the new program. 5. Sign up for online access with Cigna to have a digital copy of your insurance card.

If you are already enrolled in the dental and you are NOT adding a family member or if you are waiving ALL coverages, you ONLY need to complete the Benefit Election Form. Be sure to enroll or waive every benefit offered. If you are not currently enrolled and/or you need to add a family member to coverage you need to complete the Benefit Election Form and the proper division enrollment form.
