• We are making a carrier change effective 12/1/24 from Connecticare to Cigna. • This will affect the Medical and Dental Insurance. • You only need to complete the payroll authorization form even with the carrier change UNLESS you are adding someone to your coverage or signing up yourself for open enrollment. • The most important things to do for this renewal are: 1. Update doctors and pharmacies of your new insurance information effective 12/1/24 2. Check if any of the medications you take require authorizations with Cigna (generics for Cholesterol are no concern, expensive drugs that have TV commercials require authorizations) details on next page 3. Refill any prescriptions available for refill with Connecticare prior to 12/1 4. If you utilize the mail order program now you will need to fill out new paperwork for the new program. 5. Sign up for online access with Cigna to have a digital copy of your insurance card.

Cigna Telehealth Telehealth (also known as telemedicine) is the use of technology such as computers and mobile devices to help you get the health care from anywhere, 24/7 Members are able to talk with a doctor or nurse using Cigna Telehealth Connection Includes charges for the delivery of medical and health-related consultations via secure telecommunications technologies, telephones and internet only when delivered by contracted medical telehealth providers (more details are available on mycigna.com) Telehealth Services rendered by providers that are not contracted medical telehealth providers are covered at the same benefit level as the same services would be if rendered in-person. Making Your Elections or Getting Questions Answered What forms do you need to complete? If you are already enrolled in the dental and you are NOT adding a family member or if you are waiving ALL coverages, you ONLY need to complete the Benefit Election Form. Be sure to enroll or waive every benefit offered. If you are not currently enrolled and/or you need to add a family member to coverage you need to complete the Benefit Election Form and the proper division enrollment form. EMPLOYER PAID BENEFITS: MUTUAL OF OMAHA LIFE AND AD&D: FLAT $25,000 (*Benefit Reduction 50% age 70) BENEFIT INCREASED SHORT TERM DISABILITY: 60% to $500 WEEKLY BENEFIT INCREASED, pays AFTER CTFML benefit is exhausted when applicable.
